Deerpath Associates on Aging is a community-first eldercare organization dedicated to helping older adults remain independent, supported, and secure in their own homes. We simplify the aging journey by bringing clinical care, lifestyle services, and family guidance together under one trusted organization. Our interdisciplinary model removes the fragmentation common in traditional eldercare. Instead of navigating multiple providers, families gain a single point of coordination that prioritizes dignity, comfort, and peace of mind.

Anthony Rizzato, MHA, MSW, LSW, NHA, CDP

Founder and Chief Executive Officer

Anthony brings more than three decades of experience serving older adults and families. With advanced degrees in social work and healthcare administration, he has built a career focused on care coordination, advocacy, and aging-in-place solutions. As a licensed social worker, certified dementia practitioner, and licensed healthcare administrator, Anthony guides individuals toward resources that support independence and quality of life. His leadership is grounded in community connection, ethical care, and long-term relationships.

Emily Kocanda, PT, DPT, LMT

Director of Therapy Services

Emily is a doctor of physical therapy specializing in in-home, concierge care for older adults. Her expertise includes balance and strength training, Parkinson’s disease support, fall prevention, neurological conditions, and dementia-related care. Emily is known for building lasting relationships with clients and families, allowing for continuity of care and personalized treatment plans. Her approach emphasizes autonomy, respect, and maximizing each individual’s potential.

Our nursing services team provides high quality, conscientious support to persons that are experiencing medical challenges. Nursing advice and consultation helps our clients to better understand their diagnosis and prepare them for future needs. In a healthcare environment that is often fast paced, our team takes the time to ensure you get the personal advocacy you deserve as the patient. Our comprehensive team approach helps in coordinating care for subsequent appointments with your primary care physician or specialty care provider, attend care plan conferences and facilitate assessments with licensed providers. Our nurses also have constant contact with our licensed therapists, pharmacist, and social workers, as well as spiritual support to ensure you receive holistic care.

As a concierge provider with Deerpath Associates on Aging, she delivers personalized, in-home therapy to help older adults regain independence, preserve dignity, and improve quality of life.

Stacy Kissamis is a Licensed Clinical Social Worker with over 25 years of experience supporting older adults and their families through life’s transitions. At MPAC Healthcare and Deerpath Associates on Aging, Stacy provides psychotherapy, emotional support, and comprehensive psychosocial care tailored to the unique needs of older adults in long-term care and community-based settings.

Gina Holton

PharmD

Gina graduated from the University of Wisconsin with a bachelor’s degree in pharmacy and holds a Doctoral of Pharmacy (PharmD). Her breadth of pharmacological interventions covers community, retail, and hospital-based prescriptions. She is board certified in ambulatory care and has an exceptional depth of experience serving Northshore residents. Her work in the Pharmacology field has given her the chance to hone her skills. She has a deep passion for the clients she serves. She consistently uses her skill set to provide consultation, maximizing an understanding of the purpose of medications and the impact prescribed medications can have on a person’s wellbeing, especially as they age. Gina takes time with each person and their family to foster insight and subsequent discussions that may be essential with a client’s primary care provider. This is especially important during a transition in care or post hospitalization where medication adjustments often occur. We are so pleased to have Gina as part of the Deerpath Associates on Aging team.
